I, the undersigned, agree that I am to pay each regular monthly utility bill, plus the additional monthly installments on the arrearage balance subject to this Payment Plan. This Payment Plan allows the arrearage to be repaid in 12 equal monthly installments. Should I default on this Payment Plan by not making any required payment on or before my due date, water service will be discontinued immediately. If this occurs, then I will be required to pay the full balance due, plus any applicable charges or fees, before service will be restored. Should I terminate my service at this location before this Payment Plan is satisfied, the total remaining balance will become immediately due and payable in full. I further understand that there is no grace period for Payment Plan payments, and all monthly installments plus current balance due must be paid ON OR BEFORE my normal due date without exception or the full balance of the remaining Payment Plan and any fees will be due and payable in full prior to restoring service.
